A cash performance bond is sometimes required to be sure that Village site conditions are met and the Village right-of-way (including curbs, sidewalks, and roadways) is restored to it’s pre-project condition. Complete this form where applicable:

**Please note: When performance bonds are required they must be accompanied by a notarized statement holding harmless the Village of Martin’s Additions from any expense to complete the required repairs.
